Abstract

Introduction: Kawasaki disease (KD) is one of the most common immunological diseases and the second most common vasculitis in childhood. However, its clinical presentation can be atypical, with hydrocholecysts documented as an unusual presentation.

Objective: To describe the clinical, biochemical, and imaging characteristics of patients with KD and hydrocholecysts.

Materials and Methods: An ambispective, observational study was conducted at a tertiary care hospital.

Results: Ninety-three patients diagnosed with KD were included, of whom 8% presented with hydrocholecysts. These patients showed a higher frequency of rash, mucosal involvement, elevated levels of cholesterol, bilirubin, and fibrinogen, as well as hypoalbuminemia, possibly reflecting a more intense inflammatory response.

Conclusion: Given the limited evidence in Latin American populations, it is recommended to actively consider screening for hydrocholecysts as a possible atypical manifestation in patients with KD.
